I hereby authorize the bi-weekly deduction of the amount shown above as my contribution to the Los Angeles Police Memorial Foundation. I also authorize the Los Angeles Police Memorial Foundation to instruct the City of Los Angeles Controller on my behalf to increase or decrease this payroll deduction equal to any increase or decrease that I authorize with a subsequent payroll deduction authorization. This authorization shall be effective until cancelled in writing by me or the Los Angeles Police Memorial Foundation.
